Referrals

Referral Information

Referrals to our provisions can be made at any time throughout the academic year. Referrals to the provisions can be made by Headteachers, INCo/SENDCos etc. If your referral is suitable for a placement or outreach, your child will be accepted. Please be aware that there may be a waiting list. 


If for any reason after referral and pre-assessment, a family choses to decline the placement, the referring school will be invoiced for the time of any observations or meetings that have taken place prior to withdrawal.


To ensure a referral is justified, please use the Pre-Referral Checklist below before completing a referral form. The current cost of a Plymouth Primary Nurture placement is £1500 (£1650 from Sept 23).


Who would benefit from a Plymouth Primary Nurture referral?

  • Disengaged/disaffected pupils
  • Pupils struggling with transitions e.g. KS1 to KS2
  • Pupils at risk of repeated fixed term exclusion or permanent exclusion
  • Pupils with mental health/trauma difficulties
  • Family/social difficulties
  • Bullying/difficulty with peers
  • Pupils with complex needs including ADHD, ASC, and Attachment Disorder


Package Overview

  • 6-week programme with pre-assessment and reintegration opportunities
  • Support in a small setting
  • Highly trained, specialist teaching and TA input
  • Access to trained therapists as part of a multi-agency approach
  • Access to partner organisations e.g. Plymouth Parkour
  • Theme based learning in core and foundation subjects through a STEM curriculum
  • Access to the broader curriculum encompassing music, art, and PSHE, which provide holistic support and promote resilience
  • A 4 week parent support group from Plymouth Information, Advice and Support (PIAS)
  • Training for school staff in areas such as environment, therapeutic support and communication

Please fill out the referral form document below and send it to edison@discoveryMAT.co.uk. Referrals are made to the Plymouth Primary Nurture partnership and are then sent to specific Centres based on need.

EYFS Outreach Service (FREE)

  

We are now in a position to offer our EYFS (Nursery and Foundation) Outreach to local Plymouth schools and nursery settings. We have an EYFS SEN Teacher and EYFS SEN TA who can visit your settings to offer advice around support for children with SEMH or other special educational needs. 

This work can be for the entire setting or an individual child.
